The Service Awards for a Polio-Free World nominations are open. If you know a Rotarian who has done an
outstanding job in the area of polio eradication, please read the attached criteria, fill out the attached nomination
form and submit it by 1 November 2012. Any Rotarian can nominate any other Rotarian. Please email your
